HWS DISCIPLINE PLAN
C.A.R.E Sheets
- The front includes the C.A.R.E. Sheet report.
- The back includes the Think Sheet, which is used during the reflection conversation connected to the C.A.R.E. Sheet.
When a student receives 3 C.A.R.E. Sheets in a trimester from the same teacher/setting:
- The student will receive a Hawk Check.
- The student will be sent to the office immediately.
The principal and student will then work through the conversation together using the Think Sheet as a guide.
HAWK Check
- When a student receives 3 C.A.R.E. Sheets in a trimester OR
- A major infraction
Consequences
C.A.R.E. Sheet – consequence using classroom behavior management system (if warranted)
HAWK Check
- 1st Hawk Check
- The principal or designee will conduct a student conference using the HWS think sheet and contact parents.
- 2nd – 4th Hawk Checks
- Lunch detention and/or alternate recess
- 5th+ Hawk Checks
- One lunch detention and/or alternate recess
- After School Re-Teaching
- Major events can expedite it to after-school re-teaching, ISS, or OSS
